In a medium saucepan, combine the peanut butter, soy sauce, rice vinegar, brown sugar, garlic, ginger, sesame oil, and red pepper flakes (if using).
Cook over medium heat, stirring constantly, until the mixture is smooth and heated through.
Slowly stir in the water, a little at a time, until the sauce reaches the desired consistency.
Remove from heat and let the sauce cool.
